Summary
This study will test whether a large language model (like a sophisticated AI chatbot) can help community doctors better coordinate care for adults with high blood pressure plus either diabetes or high cholesterol. It will compare the AI-assisted management approach to standard care for 6,000 participants. The main goal is to see if the AI tool helps more people get all their related health numbers under control at the same time.
