Summary
This study is testing whether a large language model (like a smart AI chatbot) can help adults better manage multiple common health conditions. It will compare the AI-assisted care to standard community care for 6,000 people with high blood pressure plus either diabetes or high cholesterol. The goal is to see if the AI tool helps more people get their blood pressure, blood sugar, and cholesterol under control together.
