Summary
This study tests whether a structured care plan, based on patient-reported symptoms, can improve quality of life for adults with advanced cancer. Participants will either receive standard palliative care or a tailored plan that triggers specific support based on their needs. The goal is to reduce symptom burden and distress, and improve satisfaction with care.
