Summary

This study tests whether a virtual home hospital program with remote monitoring can help adults recovering from acute kidney injury spend more days at home and avoid repeat hospital visits. About 354 participants will receive either usual care or a digital health package that includes decision support tools and home-based virtual care. The goal is to improve recovery and reduce hospital readmissions.
