Can video calls keep heart patients healthier after hospital?

Summary
This study is testing whether regular video check-ins with a nurse help people with heart failure manage their condition better than traditional phone calls. Researchers will compare two groups of about 368 patients after they leave the hospital: one group gets scheduled video calls, and the other gets audio-only calls. The main goal is to see if seeing a nurse on video improves how well patients follow their daily care routines, like taking medication and monitoring their weight.
