Summary
This VA study is testing two different approaches to improve care for veterans at high risk of hospitalization. The program helps primary care teams provide more comprehensive assessments and medication coaching to better manage veterans' multiple health conditions. The goal is to find the most effective ways to keep these veterans healthier and reduce unnecessary hospital visits.
