Summary

This study looks at how social needs—like food, housing, or safety—affect the health and self-care of African American adults with both type 2 diabetes and chronic kidney disease. Researchers will talk with 330 participants to understand what helps or hinders their care. The goal is to create a culturally tailored program to improve blood pressure, blood sugar, cholesterol, quality of life, and daily self-care habits.
