Summary
This study tests a new way of delivering palliative care to 400 people with advanced heart failure or certain cancers. The care team grows as symptoms get worse, starting with a community health worker and adding a social worker, nurse, or doctor as needed. The goal is to improve quality of life and symptom control by matching care to each person's changing needs.
