Summary
This study tests a new way to help people with uncontrolled type 2 diabetes who also face challenges like food or housing insecurity. The program uses a special tool in the medical record system to connect patients with community resources and community health workers. Researchers will compare this approach to usual care in 900 participants to see if it improves blood sugar control and reduces hospital visits.
