Summary
This study is testing whether a home-based telehealth program can help coordinate care for people with serious mental illnesses like schizophrenia or bipolar disorder. Researchers will enroll 300 adults who are already receiving services from community mental health agencies to use a special device and services at home. The main goal is to see if this approach helps people manage their physical and mental health better, potentially reducing hospital visits and improving outcomes for conditions like diabetes and high blood pressure.
