Summary
This study tested a telehealth support program called GET2HOME for children with complex chronic conditions and their families after hospital discharge. The goal was to see if virtual check-ins and support could help these children avoid urgent return visits to the hospital or emergency room. Researchers also measured how the program affected the child's and family's quality of life and how quickly they could return to their normal routines.
