Summary
This study tests whether a telehealth program can help COPD patients use their inhalers correctly and avoid repeat hospital visits. About 218 adults hospitalized with a COPD flare-up will receive a virtual home visit and pharmacy education within two weeks of discharge. The goal is to improve medication use and reduce emergency visits and readmissions.
