E-Health pilot aims to keep tabs on chronically ill patients

Summary

This study tested an e-health system that lets doctors monitor patients with two or more chronic diseases (like heart failure, COPD, or diabetes) from home. The system sends alerts and coaching to help manage health. The goal was to see how well the alerts work and if patients find the system easy to use. Only 23 people took part, and the study was stopped early.
