Summary
This study tests whether a team-based program called ARTAS can help people with HIV stay connected to care after they leave the hospital. Researchers will enroll 250 adults who are newly diagnosed or not currently in HIV care. The goal is to see if this approach improves follow-up visits and reduces hospital readmissions.
