New toolkit aims to ease tough hospital transfer choices for nursing home residents

Summary
This study tests a decision-support tool called STEP for long-term care residents facing hospital transfer decisions during sudden health problems. About 200 residents, their family decision-makers, and nursing staff will use the tool and complete surveys. Researchers will compare results before and after the tool is introduced to see if it reduces confusion and improves confidence in making these difficult choices.
