This study tested a smart app called 'Weiyin Health' to help manage follow-up care for 223 adults with kidney failure who are on peritoneal dialysis. The goal was to see if the app could lower unplanned hospital readmissions and reduce infections like peritonitis. Participants used the app for reminders, AI questions, and risk alerts, while a comparison group got standard care via WeChat and phone calls.
