Summary
This study compares home-based skilled nursing care to traditional facility care for 650 adults who need rehab or nursing support after a hospital stay. Participants are randomly assigned to receive care at home or in a facility. The main goal is to see if home care leads to fewer hospital readmissions and deaths within 30 days.
