Summary
This study tested a new remote health system for adults with at least two chronic diseases like heart failure, COPD, or diabetes. The system used phone calls and online tools to monitor patients' health and provide coaching from home. The main goal was to see if the system worked properly and was easy for patients and doctors to use, with the hope it could help people stay healthier and avoid hospital visits.
