Summary
This study tests a program to help nursing home staff avoid sending residents to the hospital unless absolutely necessary. Hospital stays can be hard on older adults, so the program provides tools and training to handle health issues on-site. The study will involve 12 nursing homes and measure how often hospital visits occur, as well as staff confidence in managing care.
