Summary

This study aims to understand how a community outreach program affects the health and well-being of older adults who are often isolated and hard to reach. Researchers will follow 5,500 participants over 60 to see if the program improves their quality of life, physical function, and social connections. The goal is to gather evidence to help shape better support services and policies for this vulnerable group.
