Summary
This study tests whether a pharmacist-led program can help patients avoid medication errors after leaving the emergency department. About 82 adults who take multiple medications or are prescribed high-risk drugs will be randomly assigned to usual care or extra pharmacist support. The goal is to see if the program is practical and acceptable, so a larger trial can be planned.
