Summary
This study tests a new way to manage medications for elderly patients who take many drugs. The goal is to reduce harmful side effects by improving how doctors and pharmacists coordinate care. About 1,500 hospitalized older adults will take part, with a focus on those at high risk for drug interactions or side effects.
