Summary

This study is testing a new nursing care model designed to improve how patients with stable chronic diseases manage their health. It involves 420 patients working in partnership with an Advanced Practice Nurse to co-create a personal care plan. The goal is to strengthen the patient's role in their own care and improve their ability to manage their long-term condition.
