Summary
This study looks at whether a nurse-led program can help adults over 65 with long-term health problems better manage their own care and reduce trips to the hospital or emergency room. About 100 participants will get personalized education and at least three follow-up sessions over six months. Researchers will compare their health before and after the program to see if it improves quality of life and medication use.
