Summary
This study tested whether adding a pharmacist to a mobile geriatric team could reduce potentially inappropriate prescriptions in hospitalized patients aged 75 and older. The pharmacist reviewed each patient's medications and gave feedback to the geriatrician and attending doctor. The goal was to see if this approach lowers the number of risky prescriptions at discharge and three months later.
