Summary

This study looked at whether carefully reviewing and correcting medication lists for hospitalized patients with chronic kidney disease (CKD) could reduce mistakes and prevent them from being readmitted within 90 days. Over 300 adults with CKD stages 2-5 who took at least three regular medicines were included. The approach combined medication reconciliation with a detailed review, and results were compared to a group that received usual care.
