Summary
This study is testing whether a 12-week education and support program helps hospitalized African American adults with advanced chronic kidney disease. Participants receive education sessions, phone calls, and text messages about managing their condition and planning for future treatments. The goal is to see if this program improves patients' knowledge and confidence in managing their kidney health compared to standard care.
