New study aims to keep seniors out of the hospital after ICU stays

Summary
This study is for people aged 65 and older who have survived a critical illness and are ready to leave the hospital. It tests whether giving patients and their family doctors personalized information helps them make better decisions about follow-up care. The goal is to improve recovery and prevent avoidable hospital readmissions. About 300 participants will take part.
