Summary
This study aims to create and test a simple care plan to help heart transplant patients take their anti-rejection medicines correctly. Taking these medicines as prescribed is critical to prevent the body from rejecting the new organ. The study will involve 100 patients and will check if the care plan improves medication adherence over six months.
