Summary
This study looks at whether community health workers can help emergency department patients with high blood pressure connect to local resources that address their social needs, like food or housing. Fifteen adults with uncontrolled hypertension will complete surveys and receive referrals. Researchers will track how many stay in the program and check their medical records for one year to see if emergency visits change.
