Summary
This study looks at how combining virtual check-ins and clinic visits can help people with uncontrolled high blood pressure. About 130 adults with high readings will track their blood pressure at home and share it with their care team. The goal is to see if this approach helps keep blood pressure in a healthy range over two years.
