Summary
This study is testing whether better communication between a hospital pharmacist and a patient's local pharmacy can make going home from the hospital safer. It focuses on adults taking seven or more medications. Researchers will compare the usual process to a new one where pharmacists collaborate directly during discharge to see if it reduces medication mix-ups.
