Summary
This study compares home visits to standard clinic visits for managing high blood pressure and type 2 diabetes. Researchers will track changes in blood pressure and blood sugar over 6 months in 100 adults aged 18-60. The goal is to see if home-based care leads to better control of these chronic conditions.
