Summary
This study tests whether nurse home visits can help people with high blood pressure and type 2 diabetes manage their conditions better than standard clinic visits. Researchers will measure changes in blood pressure and blood sugar over 6 months. The study enrolls 100 adults aged 18-60 who speak English or Spanish.
