Summary

This study tests a nurse-led home care program for people with advanced heart failure and their family caregivers in rural Appalachia. The program uses faith-based nurses and local volunteers to improve quality of life and reduce symptoms. About 208 patients and their caregivers will take part. The goal is to see if this approach helps patients feel better and supports their families.
