Summary
This study tests whether showing a video about advance care planning at home helps people with life-limiting illnesses understand hospice care and document their medical wishes. About 500 adults with a one-year prognosis or less will be randomly assigned to watch the video or receive usual care. The goal is to see if the video increases hospice use and improves how patients' preferences are recorded.
