Summary

This study is testing whether a program called Heart-to-Heart can help more people from low-income backgrounds start and stick with cardiac rehabilitation after a heart event. Participants will work with a community health worker over several coaching sessions for support and education. The goal is to see if the program is helpful, easy to use, and if people would recommend it to others.
