Summary
This study is testing a new digital health platform designed to help people with early-stage Cardiovascular-Kidney-Metabolic (CKM) syndrome. The platform will monitor health data, provide personalized diet and exercise plans, and offer remote consultations. Researchers will enroll 150 participants to see if using this platform for two years improves their health outcomes and quality of life.
