Summary
This study tests a program called Food as Medicine (FAME) that helps families at risk for chronic diseases get and eat healthier food. About 250 children aged 8-15 and their caregivers will get help from a food navigator to connect with community food resources. The goal is to see if this improves diet quality, food security, and health over 12 months compared to usual care.
