Summary
This study tests whether a personalized exercise program called VIVIFRAIL can improve strength, balance, and mobility in frail adults over 65 who are in an intermediate care hospital. Participants will do exercises tailored to their ability, and researchers will track their progress for six months after discharge. The goal is to see if this approach can prevent further decline and improve quality of life.
