Summary
This study tests an app called ENHANCe that helps family doctors manage high blood pressure using home blood pressure readings. About 116 patients and their doctors will use the app to see if it improves care and blood pressure control. The goal is to make it easier for doctors to make treatment decisions without needing an office visit.
