Summary
This study will test a new care management checklist designed to guide end-of-life care for people with advanced dementia who are receiving home healthcare. Researchers will work with 96 participants, including patients, their care partners, and healthcare professionals, to see if the checklist is practical and helpful. The goal is to improve the transition to hospice care and increase satisfaction for families.
