Summary
This study tests whether sending a letter before a doctor's visit can encourage older adults and people with serious illnesses to choose a healthcare proxy—someone to make medical decisions if they can't. About 10,000 participants will either get a letter or not, and researchers will compare how many officially name a proxy. The goal is to find simple ways to help more people plan ahead for their care.
