Summary
This study tests whether coaching can help people with dementia and their caregivers after an emergency room visit. About 19,200 participants aged 66 and older will receive different combinations of support to connect them with community services. The goal is to improve care, reduce stress, and lower the number of future ER visits and hospital stays.
