Summary
This study aims to improve care for people living with dementia and their caregivers after a visit to the emergency room. It tests different coaching programs to help connect families with community support services. The goal is to improve the transition from the hospital, increase satisfaction with care, and reduce the need for future emergency visits and hospital stays.
