Summary

This study tests whether adding a digital lifestyle program to standard care helps adults with type 2 diabetes who are at high risk for complications. About 200 participants will use apps that support healthy eating, activity, and weight loss, while others receive usual care. The goal is to see if the program improves weight, blood sugar, and quality of life over 12 months.
