Summary
This study is testing whether a doctor-led lifestyle counseling program, combined with two personalized smartphone apps, can help older adults who have received liver or kidney transplants improve their eating habits and physical activity. Participants first receive medical advice on diet and exercise, then use the apps at home for 12 weeks while researchers track whether they stick with the program and if it leads to healthier behaviors. The goal is to see if this digital-supported approach is practical and helpful for transplant recipients who need to maintain their health long-term while taking ongoing medications.
