Summary
This study tested a telephone-delivered program for 100 African Americans aged 50 and older with poorly controlled type 2 diabetes. The program combined diabetes self-management education with help resolving unmet social needs, like food or housing. Participants were randomly assigned to the program or usual care, and researchers measured changes in blood sugar levels and patient well-being.
