Summary
This study tested a new digital tool designed to help doctors and pharmacists safely reduce the number of risky medications taken by older adults in the hospital. Many common medications can cause side effects like falls, confusion, and memory problems, especially when taken together. The tool automatically flags these medications so the care team can review them and potentially stop, lower the dose, or switch to safer alternatives.
