Summary
This study tests a program to help older adults (65+) safely reduce or stop unnecessary medications that may cause side effects. Researchers will use a risk prediction tool to identify 100 participants and compare those who receive the deprescribing program to those who don't. The goal is to see if the program reduces hospital visits and lowers the number of medications people take.
