Summary
This study is testing whether a new support program can improve quality of life for people with advanced COPD. The program includes personalized needs assessments, future care planning, and virtual team meetings. Researchers will compare 100 participants receiving this support against a group receiving usual care to see if it reduces hospital visits and improves well-being.
